# Bulimia and Binge-Purge Cycles

Binge-purge cycles can feel impossible to talk about. Here, we approach them with curiosity rather than judgment, and with care for your body as well as your story.

Bulimia often lives in secret. From the outside, everything may look fine. Inside, there can be a relentless loop: restricting, then bingeing, then compensating through purging, fasting, or exercise, followed by shame and a promise that this time will be the last. Over time, the cycle can come to feel like the only way to manage what's happening inside.

If this is your experience, there is nothing shameful about reaching out. ## What the pattern may be doing for you.

The binge-purge cycle often serves more than one purpose. You may recognize some of these:

- Bingeing as a way to soothe, numb, or escape overwhelming feelings.
- Purging as a way to release tension, undo, or regain a sense of control.
- Restriction that sets up the next binge through physical and emotional deprivation.
- Shame and secrecy that keep the cycle hidden and hard to interrupt.

Seeing the pattern this way doesn't excuse the harm it causes. It helps us understand why willpower alone hasn't been enough, and it points toward what might need care instead. ## Physical safety comes first.

Bulimia can affect the body in serious ways, sometimes without obvious outward signs. Because of that, medical care is an important part of recovery. When it helps your care, I'm glad to coordinate with a medical provider and a dietitian so your physical health is looked after while we do the emotional work.

If your physical health needs more support than weekly outpatient therapy can offer, a higher level of care may need to come first. We can talk about that openly, without shame, and plan for what comes next.

How we work

## Curiosity, compassion, and pacing.

This is a weight-inclusive practice. You won't be weighed, asked to track numbers, or measured against an ideal. Instead, we look at the cycle from the inside, with compassion for every part of you involved in it.

Parts-centered work (https://braveemberswellness.com/approaches/parts-work) helps us understand the part that binges, the part that purges, and the critic that shames you afterward. Somatic work helps you notice and tolerate the sensations and urges that drive the cycle. Exposure and Response Prevention (https://braveemberswellness.com/approaches/exposure-and-response-prevention-therapy-erp), always paced and never used on its own, can help your system learn to ride out urges without acting on them. When painful experiences sit underneath, EMDR may help those memories lose their charge.

## Individual work and group work.

I work with adults 21 and older, in person at my Doylestown office and by secure telehealth. Individual therapy is usually the first step. ## What people ask.

Do I have to stop purging before I start therapy?

No. You don't need to have stopped any behavior to begin. Therapy is a place to understand the cycle and build support for change, at a pace that's safe for you. If your physical health needs closer attention, we'll talk about medical support too.

Is bulimia only about food and weight?

Usually not. The binge-purge cycle often connects to feelings, stress, past experiences, or a need for control or relief. Food and body image matter, and we talk about them, but much of the work focuses on what's underneath.

Can bulimia therapy happen by telehealth?

Yes. Secure video sessions are available, as well as in-person sessions at my Doylestown office. With telehealth, a private space where you can speak freely matters, and we'll also make sure your medical care is in place.

What if therapy for bulimia hasn't helped me before?
